The First Bowl
And I saw another great and wonderful sign in heaven: seven angels, having seven plagues—the last, because in them the wrath of God was completed. – Rev.15:1
Then I heard a loud voice from the temple saying to the seven angels, “Go, pour out the seven bowls of God’s wrath on the earth – Rev.16:1
The first angel went and poured out his bowl on the land, and ugly, festering sores broke out on the people who had the mark of the beast and worshiped his image. – Rev.16:2

As the scripture states, the first bowl only affects those who will have received the mark of the beast and worshiped his image.
The Greek “Kakos” here in the verse means “a noisome, grievous sore,” noisome meaning – bad, foul and the Greek “poneros” meaning to cause pain, i.e. bad, foul, painful sores.
There are some who interpret this sore breaking out as a result of it being a foreign object within the body and therefore the body producing these sores as a reaction, which is a possibility. But, the scripture also mentions that the reason that these sores will break out upon people, is because they worship the image of the beast. Therefore, the sores may not just be breaking out as a result of the body reacting to a foreign object, but may be caused directly by God as a result of both receiving the mark and worshiping the image of the beast.
Being that it is stated that the sores break out upon those who receive the mark of the beast and those who worship his image, then this bowl judgment will neither affect Israel who will have fled out in the desert to that place prepared for her by God, nor the great tribulation saints (Gentiles) of whom it is said that they will not have worshiped the beast, his image nor received his mark. Therefore, these two groups will be exempt from this plague of wrath.
As it is with the 5[SUP]th[/SUP] trumpet, so also is it with the first bowl judgment which is specifically to torment, for there are no fatalities mentioned as a result of these noisome, painful sores.
