I'm gonna say no.

Changing my name, social security number, place of living, etc. doesn't change who I am or change my proclamation of salvation and my baptism.

If that were the case, I'd have to get re-baptized every time I moved, or after I got married. I don't see the point.

Just because everything else has changed, doesn't mean your heart has. It is not necessary to have to do it again.
