No, you are right, it has to be the intention. There are some guys named Dick, like Dick Clark. But if you called someone that who didn't have that name it then becomes a swear word. Like you said, same as donkey and ass. "Do not let any unwholesome talk come out of your mouths, but only what is useful for building others up." Your conscience should tell you what is unwholesome.
