Q1) Are you saved? When? How do you know?
Q2) What must one do to be saved (if there is anything they must do)?
Q3) When did you receive the Holy Spirit?
Q4) What is the evidence that you have received the Holy Spirit?
Q5) How were you baptized (immersion in water, sprinkling of water, fire, some other way) ?
Q6) If you weren't baptized in any of the above ways (Q5), why not?

Yes. 1988. Because the Bible tells me that if I believe, I am saved.(John 6:47)

Q2) What must one do to be saved (if there is anything they must do)?
Believe, repent, be baptized, do his Will

Q3) When did you receive the Holy Spirit?
At baptism (Acts 2:38)

Q4) What is the evidence that you have received the Holy Spirit?
No subjective evidence, but the objective assurance of the Promises of God regarding the Holy Spirit.

Q5) How were you baptized (immersion in water, sprinkling of water, fire, some other way) ? immersion in water

Q6) If you weren't baptized in any of the above ways (Q5), why not?
