There is no crying in gym. It is always a bit upsetting at first when you are out of shape and step on the scale or measure your bodyfat. Its a journey , if your wife wants to be happy about her body tell her it takes hard work and determination and be there for her , work out together and motivate her to push herself. Takes time bro.

Perhaps this was a passive-aggressive way for you to let her know you think she is fat? I suspect that since you are in the healthcare field, "and see first hand the results when people eat poorly, never exercise, and don't take care of themselves", that she hears this from you quite a bit. Just a guess based on her extreme emotional reaction. Regardless if this is true or not, she is probably feeling judged and unlovable for not being in perfect condition. The best you can do is tell her you love her "just the way she is", and remind her of all the reasons you fell in love with her.
