I have tried the 'moving and getting a fresh start' thing. I was a little older than you when i tried it. Just keep in mind its not a magical transition that you will suddenly become a new person just from moving. I don't say this to discourage you, but to help you keep from getting discouraged in the future. And this isn't to say that there can't be some changes when you get a fresh start. There can be, you can try and do some things to change yourself and how others view you. But they will not be drastic.
Keep in mind, when you move, you are still you, and many of your problems will follow you. You aren't going to leave all sorts of things behind. So changes can and will happen, just don't get a huge expectation that the changes will be drastic. But more like a 'get the ball rolling'.
And a friendly suggestion. When writing long posts, make sure to add more space breaks. You have a 'wall of text', which will prevent some people from even attempting to read your post, and thereby possibly getting less responses.
That said, i skimmed your post, trying to get some ideas of what it said. I have battled anxiety, and known others who had it so bad they didn't like to leave the house either. I have, and still, struggle with self worth and feeling i have anything to offer. So you aren't alone in what you're going through.
