Title should be either:

All hell HAS broken loose (past participle uses helping verb have)

OR

All hell IS Breaking loose. (present imperfect)

You simple cannot put “is” and “broken” together, as that is combining present tense and past tense. Besides the fact that it is totally unclear!

I have my doubts God told you to use bad grammar on CC!
