Put your thumb on the table, only the outer joint. Place the bottom part of the plate towards it. same with the knife and fork,
they can be on one side each (knife on right) or both on the right side. glass just above the knife.
That is very good, Astrid, but first you need to wash you hands and put on some freshly laundered white gloves.

also, the plate should be warm if dinner is served, and the cutlery always newly polished with a soft cloth so they maintain their shine

We live in a city so small, that we're actually advised to drink bottle water! haha

I also was concerned about Astrid’s reference to tap water.

From now on we will not allow tap water into our houses. We will use bottled water for all things, including cleaning the house.
