It means exactly what it says. To believe in your heart that Jesus is Lord is to believe everything that is written about Him, particularly His finished work of redemption.
I disagree. Only Christians can confess Jesus is Lord. The lost must confess the Lord Jesus, who He is and what He came to do. The NIV is wrong.

Here is what we read in the KJV: That if thou shalt confess with thy mouth the Lord Jesus, and shalt believe in thine heart that God hath raised him from the dead, thou shalt be saved.

Here is what it means (Gill's commentary):
That if thou shalt confess with thy mouth the Lord Jesus,.... That is, if a man shall make a good, sincere, and hearty confession to God, before the church and people of God, and before the world, that Christ is his Lord and Saviour, whom he desires to serve, and to be saved by; and this as arising from a comfortable experience of the grace of God in his soul, and from a true faith in Christ in his heart, wherefore it follows...

When you confess another, you confess the testimony about them. Confessing Jesus Christ is confessing who He is and what He came to do.

Romans 10:9 That if thou shalt confess with thy mouth the Lord Jesus, and shalt believe in thine heart that God hath raised him from the dead, thou shalt be saved.

The word "confess" is the Greek word homologéō which means to say the same thing as another, i.e. to agree with, assent.

What the verse means is that we read and/or hear the Word of God concerning Messiah, the Lord Jesus Christ, and we are in agreement with what is declared to us through our reading or through what we hear from others.

God proclaims Messiah and we agree that Jesus Christ is Lord and believe in our heart that God raised Christ Jesus from the dead.

We then preach the gospel to others because it pleased God by the foolishness of preaching to save them that believe (1 Cor 1:21). But it is not our preaching which saves us.

What saves us is the faith which was stirred up within us when we "heard" the gospel.

God tells us Jesus Christ is Lord. God tells us Jesus Christ was resurrected from the dead.

We agree (confess with our mouth) that Jesus Christ is Lord and we believe (in our heart) that God raised Jesus Christ from the dead.

We are then saved. The word "saved" is the Greek word sṓzō which means deliver out of danger and into safety; used principally of God rescuing believers from the penalty and power of sin – and into His provisions (safety).

My Interpretation of this scripture
If a person Is not truly repenting then they can't come to GOD.

Here's a thought I had...is confess with thy mouth talking about the literal mouth since believing with the heart is not the literal heart? The heart of man is the inner man, the soul. Is verbal communication necessary or inner prayer?
I don't think it is factual to say there is a separation between what we say and do and what is in our hearts.

All scripture is from the spiritual God who lives in a world of the spirit, not a world of the flesh. So God speaks to us through the flesh, and what is of the flesh has a spiritual side as well as a fleshly side. To sin is to spiritually die. It takes a fleshly confession of sin that reflects our spirit to spiritually live.

I think the people who are living in a purely physical world without any acknowledgment of the spiritual are living in a false world, for what they do reflects the spirit.

Lord = My Master (Greek translated "Adon-kai (Nai) from Hebrew which means my Master)

No person can accept the truth unless God works in their life for the flesh on it's own would never accept the truth for there is no good things that dwells in the flesh, and the Spirit and flesh are contrary to each other.

1Co 1:26 For ye see your calling, brethren, how that not many wise men after the flesh, not many mighty, not many noble, are called.

Many are called but few are chosen, and when they are called God will work with them but not all make it to the truth of what Christianity is about.

God will call us according to our heart condition which the ones that were not called were too self exalting with a lack of love for people and liked positions of power over people.

2Ti 3:4 lovers of pleasures more than lovers of God;
2Ti 3:5 Having a form of godliness, but denying the power thereof: from such turn away.
2Ti 3:7 Ever learning, and never able to come to the knowledge of the truth.

Mat 7:21 Not every one that saith unto me, Lord, Lord, shall enter into the kingdom of heaven; but he that doeth the will of my Father which is in heaven.
Mat 7:23 And then will I profess unto them, I never knew you: depart from me, ye that work iniquity.

Which many can confess Christ as Lord for God was working with them when He called them but they fell short of what Christianity is about in the truth.
