26 Therefore if the uncircumcision keep the righteousness of the law, shall not his uncircumcision be counted for circumcision? 27 And shall not uncircumcision which is by nature, if it fulfil the law, judge thee, who by the letter and circumcision dost transgress the law? 28 For he is not a Jew, which is one outwardly; neither is that circumcision, which is outward in the flesh: 29 But he is a Jew, which is one inwardly; and circumcision is that of the heart, in the spirit, and not in the letter; whose praise is not of men, but of God.

True physical circumcision is just a little *snip* off of the most sensitive loose skin that hangs over the very end.
The american medical industry butchered it to mean taking the whole skin off, so they can sell the skin to make face cream.

Physical circumcision is more hygienic than uncircumcision, so it symbolizes a heart cleansed from the defilements of the flesh.
During Hellenist times Jews used to undergo circumcision reversal, epispasm, to avoid being laughed at while doing gymnastics nude. Being naked in public was OK, but exposing the glans was considered to be indecent (whew! glad they weren't completely naked!). That may be what Paul was referring to here

So before a person is born again their sinful nature is always turned on and can't turn it off. After born again God gives you the ability to finally turn it off.
