If Democrats take back a majority in the House, they can call a vote for impeachment. If a majority votes for impeachment, then it goes to the Senate where they vote whether to convict or not. If a two-thirds majority votes to convict, the president is removed and replaced by the vice president. The Senate can't send him to prison but after he's out of office he can still be prosecuted. The likelihood of him getting convicted is pretty low though since the supreme court basically gave him a get-out-of-jail-free card for official acts done as president.
If Democrats can't get a good majority in the Senate, I doubt the House will even try to impeach; it would just be a symbolic gesture and a waste of time. He's already been impeached twice and it did nothing to stop him.
