Yeah. Larger cities usually have better hospitals.

Not that big difference here between the local and the major ones, apart for specialied stuff here

why is it so differnt there? is there no top- level control of some kind?

Top rated doctors don't want to live in a small town. Most specialists we have to drive to Denver or Grand Junction to see.

That's cus the best hospitals are in the larger cities and that's also where the money is.

We have high taxes, but they're not usually used for hospitals.

high taxes? hmm, not the impression I got.

(we have income tax of about 30%,
food tax (varies a bit, 5- 15 I think), property tax, "culture tax" (yeah, we pay 15% VAT on cinema tickets :p )

what happes to your tax money then?

The government mostly takes it to pay their ridiculously high salaries.
