why are some new threads held up for review?

Not some new threads, the vast majority of new threads are reviewed prior to going public. Only a handful of long term users threads bypass the review process.

As far as how long, anywhere from a couple of minutes to 10 or 12 hours depending on when one of us has time to log in and clear the que.

That would probably have to be reviewed first. That process could take a couple of minutes to 10 or 12 hours. At least that is my understanding.
